Community Care, Inc. is seeking a compassionate LPN to provide one-on-one private duty nursing for a 32-year-old female adult in Uniontown, PA. The role supports trach/vent care, administers medications, and helps with activities of daily living in a home setting.

This full-time position offers evenings or overnights, 1:1 care, weekends available, and a supportive team that values caregivers and quality care.

Qualifikationen

• Current LPN license in PA, or RN license in PA.
• Graduate of an accredited nursing program.
• At least 6 months of nursing experience (home health preferred).
• CPR certification required.

Aufgaben

• Assist with Activities of Daily Living (ADLs).
• Perform trach/vent care and monitoring.
• Administer medications safely and accurately.
• Communicate clearly with the care team and family.
• Deliver patient-centered, family-inclusive care.
