Centria Healthcare is seeking a compassionate nurse to provide 1:1 care for an 85-year-old female with complex medical needs in a family home in Michigan. The role requires managing trach care, catheter care, and total care with clear communication with a large, involved family.

Qualifications

• Active RN or LPN license in Michigan.
• Home care or private duty experience preferred.
• Trach/vent experience is a plus and training is provided.
• Strong communication and critical thinking skills.

Responsibilities

• Provide skilled nursing care (trach/vent care, medication administration, catheter care).
• Monitor and document patient condition and vital signs.
• Assist with daily living activities (ADLs).
• Communicate with families and care teams.
• Maintain safe, high-quality, patient-centered care.
