Qualifications

• LPN/LPT/LVN license active in state of practice and residence
• Valid driver's license with acceptable driving record and proof of insurance
• CPR certification (professional)
• 1+ year nursing experience preferred
• Home Health care experience preferred

Responsibilities

• Provide 1:1 care per client’s plan of care.
• Administer medications, feedings, oxygen, ostomy care as needed.
• Document nursing actions and communicate with family, case manager, and physicians
• Coordinate or order supplies, medications and feedings as needed
• Ensure patient needs are met per physician’s instructions
