Job summary

BrightStar Care seeks a Licensed Practical Nurse (LPN) to provide one-to-one home care to patients in their homes. You will carry out the plan of care and document nursing activities with accuracy and timeliness.

The role requires an active LPN license, at least one year of nursing experience, strong problem-solving and organizational skills, and compliance with health and safety guidelines. A valid driver’s license, auto insurance, BLS CPR, and up-to-date TB test or chest X-ray are also needed.

Qualifications

• Active LPN license in the state where care is provided.
• Minimum of 1 year nursing experience.
• Strong problem-solving and multi-tasking skills.
• Knowledge of health and safety guidelines and procedures.
• Valid Driver's License, Auto Insurance, BLS CPR, and an up-to-date TB skin test or Chest X-Ray.
• Must be able to stand, walk, squat, bend, kneel, reach, twist, push, and pull.
• Must be able to lift or transfer up to 50 pounds.

Responsibilities

• Provide 1:1 care in the patient's home.
• Implement treatment and plan of care using sound judgment and critical thinking.
• Maintain accurate, detailed, and timely documentation consistent with the care plan and nursing processes.
• Perform all skills, treatments, and procedures to the scope of practice defined in the Nurse Practice Act in the state of clinical practice and adhere to the employment agency's policies, procedures, and protocols.
