Medical Director Physician

The Personalized Health Partners medical practice is currently recruiting a physician to serve as Medical Director covering the Indianapolis, IN region.

As Medical Director, you will be part of an innovative care model combining excellent bedside manner with advanced technologies to reduce hospital readmissions and improve patient outcomes.

Job Duties and Responsibilities

1. Assume administrative authority and responsibility for implementing medical services, policies, and procedures in accordance with regulations and current practices in post-acute and long-term care medicine.
2. Coordinate medical care, including emergency procedures, maintain effective communication with attending physicians, and ensure quality of care is maintained.
3. Provide physician services within skilled nursing facilities and/or clinics.

Qualifications

• M.D. or D.O. required
• Gerontology certification preferred
• MD or DO license in Indiana
• ABMS Board Certification in Internal Medicine, Family Practice, Geriatrics, or Hospice/Palliative Care
• 5+ years of experience with geriatric or long-term care populations
• Experience working with and educating NPs (preferred)

Preferred Qualifications

• Healthcare management/leadership experience
• Familiarity with CMS, Medicare Advantage, SNP regulations
• HCC and Risk Adjustment methodologies
• Utilization metrics and bed management
• State-specific collaborative practice agreements and licensing
• Knowledge of EMR systems
