Basic Requirements:

• Min Bachelor’s degree in Healthcare Administration, Business Administration, or a related field

• Experience in community services, project management, or healthcare operations

Key Responsibilities:

1. Community-Oriented Clinical Operations & Business Partnership

• Refine and develop integrated care models (transitional, pre-, post-hospital) to improve patient outcomes

• Manage and analyze data to support decision-making

• Lead and manage quality improvement and innovation projects to improve team effectiveness and efficiency

2. Project Management and Departmental Support

2.1 Project Management & Support

• Operationalize integrated care models between hospital and community

• Provide support to Alexandra Hospital teams delivering home- and community-based care

• Drive and support improvement projects using methodologies like Agile, GROSS, and GROW

• Assist in marketing programs to community partners and residents

• Organize and manage community events when required

2.2 Programme Evaluation & Improvement

• Conduct surveys and collect data for program evaluation

• Use data analytics or collaborate with analysts to generate reports to support decision-making

2.3 Collaborations & Relationship Building

• Build partnerships with healthcare providers in the community

• Support initiatives with nursing homes and strengthen ties with primary care partners
