Temp/Contract Healthcare Assistant (5-5.5 Days a Week, West, East) #HYNT

We are seeking a Healthcare Assistant to be outsourced to various healthcare organizations across Singapore!

• Build strong rapport with internal teams and external stakeholders, including patients and caregivers.
• Support preventive health screenings and vaccinations aligned with national health programmes.
• Help reduce hospital readmissions and escalations to tertiary care through timely interventions.
• Gather and apply insights from social determinants of health to guide care planning.
• Use health IT systems to track, document, and report care activities and clinical indicators.
• Perform basic health assessments (e.g., blood pressure, glucose, weight monitoring).
• Provide basic health education on lifestyle, nutrition, physical activity, and social prescribing.
• Promote health literacy by introducing patients to digital tools and community resources.
• Encourage adherence to recommended vaccinations and health screenings.

Job Requirements

• Diploma in any discipline.
• Strong communication and interpersonal skills.
• Passion for preventive health and community outreach.
• Comfortable using technology to support patient care and education.
