Job description

Job purpose

You are responsible for supporting the Dialysis Centre in delivering holistic care by assisting in all aspects of management to achieve maximum efficiency and cost-effectiveness in the facility, while maintaining a safe environment that supports optimal patient care.

Duties and responsibilities

1. Administrative Responsibilities

  • Record and update all the patient’s relevant medical history and related data, and ensure confidentiality of information

  • Participate in cost control initiatives

  • Ensure that the facility is in a clean, safe and sanitary environment

  • Involved in stock inventory

  • Assume other additional duties and responsibilities assigned by the Nurse Manager

Staff Related Responsibilities

  • Ensuring that the centre has enough available consumable stocks

  • Incident reporting to the Nurse Manager (relating to clinical incidents, technical issues, patients' complaints, compliance issues, etc.) promptly

  • Join the handover meeting from the morning team to the afternoon team

  • Supports the team for any quality improvement projects as assigned by the Nurse Manager

  • Assist in training the cleaner

Clinical Duties

  • Demonstrate competency in dialysis-related procedures, including the care and procedure of dialysis access such as cannulation of arterio-venous fistula (AVF) and arterio-venous graft (AVG) and preparation of central venous catheter (CVC) based on prescription and according to the expected standards of practice and guidelines.

  • Preparation, commencement and completion of dialysis treatment, management of dialysis complications, responsible for clearing dialyzers and bloodlines after dialysis, etc.

  • Responsible for assessing and documenting patients’ parameters and psycho-physio-social needs before, during and after treatment. Report any abnormalities to the Senior Staff Nurse promptly

  • Demonstrate competency in the collection of a patient's blood sample,

  • Demonstrate competency in dialysis equipment-related procedures according to protocol, including troubleshooting, disinfection, care and maintenance.

  • Practice primary nursing and treatment plan for assigned patients in collaboration with the Senior Staff Nurse.

  • Assist the Senior Staff Nurse during the Doctor consultation round.

  • Ensure patients and their families are educated on renal dialysis care by regular communications

  • Ensure accurate, complete, and timely documentation of patients’ clinical notes, which includes care provided.

  • Provide updated blood test results on a timely basis to ensure accurate and timely reporting of the Renal Team’s Clinical Outcome and Quality Measures.

  • Participate in quality improvement projects to pursue quality outcomes for patients.

  • Adhere to proper aseptic techniques and infection prevention and control protocol.

  • Coordinate with external care provider for continuous patient care e.g., medical appointment, ordering of the medications on online platform, referring to the social worker, etc.

Qualifications and Requirements

  • At least a Diploma in Nursing

  • Valid registration with the Singapore Nursing Board

  • Minimum 2 years of renal nursing experience

  • Valid certificate of Basic Care Life Support (BCLS)
