Sr Pharmacist

This job description details the responsibilities and the general duties of the Drug Information Pharmacist.

MAJOR DUTIES & RESPONSIBILITIES

1. SPECIFIC (50%)
2. Provide drug information and clinical advice to other healthcare professionals and patients.
3. Assist in the preparation and administrative work for all Pharmacy and Therapeutics (P&T) Committee meetings, including:
4. Writing drug evaluation reports to justify formulary revisions.
5. Analyzing and reporting data on drug utilization patterns or adverse drug reactions.
6. Communicating formulary revisions to other healthcare professionals.
7. Work and coordinate with intra- and inter-department stakeholders to provide smooth and safe flows of medication use within the hospital.
8. Participate in drug information/medication safety activities as required by Pharmacy.
9. Assist in training of pre-registration pharmacists and pharmacy students in the drug information section.
10. GENERAL (50%)
11. Conduct clinical review for appropriateness of medication orders.
12. Dispense medications to patients, explaining the indications for drug therapy and instructions for administration.
13. Other administrative work or duties as directed by the supervisors.

JOB REQUIREMENTS

Education / Training / Experience

1. Bachelor of Science (Pharmacy) from an accredited University recognized by the Singapore Pharmacy Council.
2. 2-3 years of working experience in a hospital setting is preferred.
3. Registered pharmacist with the Singapore Pharmacy Council.
