Programme manager responsible for Project Renalssance which aims to demonstrate that early introduction of palliative care options for this irreversible disease will be effective, scalable, economically sustainable and have a significant impact on the quality of lives of advanced CKD patients.

Job Responsibilities

1. Responsible for all operational aspects of the project.
2. Developing and driving the implementation of Kidney Supportive Care clinic for multidisciplinary management of advanced CKD patients (under supportive care).
3. Disseminate and implement hospital’s policies and procedure; develop, review, and update the team’s standard operating procedures (SOPs), protocols, and case management tools.
4. Mentor Case Management officer on project management / staff performance management.
5. Precept newly joined renal coordinators in delivery of specialty care where needed.
6. Monitor and evaluate staff performance with due follow-up on recommendations for improvement.
7. Assuring that educational opportunities are made available for staff so that they can develop professionally and contribute to the growth and development of the Division.
8. Work alongside CMO to assist with overall coordination of appointments to Kidney Supportive Care Clinic and referral to community partners (Home hospices/ integrated home health etc.)
9. Apply understanding of prevention of medico-legal issues in relation to patient care.
10. Participate in quality improvement activities.
11. Take on additional responsibilities or tasks as assigned by the Head of Division.

Job Requirements

1. Degree in Nursing, Biomedical Science, Life Sciences, Biotechnology or General Science.
2. A certificate / diploma in Renal Nursing will be advantageous.
3. Registered Nurse with at least 3 years of clinical experience.
4. Must have at least 5 years prior working experience in a healthcare environment and have assumed some managerial duties before.
5. Knowledge of clinical skills and critical thinking in delivery of patients’ care will be advantageous.
6. Must demonstrate the ability to resolve problems in all the aspects of the implementation of the Project Renalssance including all operational issues, IT issues, clinical related issues etc.
7. Must be able to lead a team of Coordinators in the implementation of the programme to deliver all key performance indicators, motivate staff and ensure knowledge retention and succession (if necessary).
8. Possess good interpersonal and communication skill.
