A social welfare organization in Singapore is looking for a Social Worker to conduct assessments of clients and provide counseling services. The ideal candidate holds a degree in Social Work and has 2-5 years of experience in healthcare, preferably in disability work. This role involves documenting case histories and ensuring effective communication with various Ministries. The organization offers a supportive working environment committed to professional growth.
Assist to conduct an assessment of clients and family members in their homes before recommending admission to the Home. Conduct an assessment of severity, key risks, and protective factors for intervention, if necessary, Home Visit, and provide counseling services to help clients and family members to manage issues associated with their disabilities to enable some degree of independent living. Conduct an assessment of the client or family and submit a social report with action plans based on the assessment of the client and family. Prepare case history information, psychosocial assessment, and social work treatment plans. Document and maintain case files and prepare reports according to the required documentation guidelines and meet the Ministries standard. Conduct closure of cases upon achieving sustainable service outcomes for clients. Plan, develop and implement curriculum, psychoeducational training and group work for clients and or their caregivers. Participate in developmental programs and supervision for personal and professional growth and development. Maintain minimum requirements for renewal of membership with the professional association. Working and reporting to Ministries. Administer financial assistance and or seek deviation for clients from the Ministries to allow the clients and their families to have ease in their finances support to the client stay in the Home. Network with other agencies to tap on community resources to provide intervention support to clients. Work with therapists and training officers in planning for client individualized care plans. Assist programme head or supervisor with programme accountability reporting duties.
Evaluate the effectiveness of the training or group work interventions.
Hold a degree in Social Work from recognized institutions with 2-5 years of experience in a healthcare setting. Manage good relationships and communication with various Ministries. Have excellent communication and interpersonal skills with demonstrated ability to build relationships. Preferably have a minimum of 3 years experience in disability work. 5.5 days work week.
