Lakeside Family Services is seeking passionate individuals to join the Youth Community Outreach Team, focused on raising awareness of youth mental health issues. Qualifications

  • Minimum 2 years of experience working with youth, preferably in community care or health care.

Job description

The Youth Community Outreach Team (CREST-Youth) aims to raise awareness of mental health issues and promote early identification among youths aged 12 to 25. This is done through outreach events, engaging parents and peers in their recovery journeys, and linking them to suitable health and social support networks.

Responsibilities:

  • Lead Outreach & Education Initiatives
    -Organise community and school-based outreach activities (e.g. workshops, roadshows, talks) for students, parents, and educators
    - Raise awareness of youth mental health issues
  • Promote Early Recognition
    -
    Conduct screening initiatives and public education to encourage early identification of emotional and mental health concerns
    - Facilitate timely access to appropriate help
  • Provide Support & Guidance
    -Offer basic emotional support to youths and caregivers
    - Assist in navigating mental health services and community resources
  • Facilitate Referrals & Follow-ups
    -Coordinate with mental health and social service providers to ensure appropriate support
    - Monitor progress and engagement through follow-ups
  • Strengthen Community Ties
    - Build and maintain partnerships with schools, youth organisations, and healthcare providers
    - Promote coordinated mental health education and service delivery
  • Monitor & Evaluate Programmes
    -Support data collection, track outcomes, and evaluate outreach programmes
    - Use insights to enhance programme effectiveness and relevance to community needs

Requirements:

  • Degree or post-graduate diploma in Social Work, Counselling, Psychology or Degree or Diploma in Occupational Therapy or Nursing or Master’s Degree in Clinical Mental Health
  • Minimum 2 years of experience working with youth, preferably in community care or health care.
  • Organised and detailed oriented in planning youth services programmes to achieve desired outcomes
  • Good interpersonal and communication skills in working with youths-at-risk
  • Willing to work on weekday evenings and/or weekends when required.
