Job Description

The Social Worker is qualified and trained to provide interventions and programmes, perform community development work and uplift the professional practice in social work. He or she conducts casework and group work as well as implement and evaluate community development activities and programmes to address service gaps in the community. A team player who is committed and service-oriented, he or she works as part of a collaborative and interdisciplinary team. He or she also works in collaboration with other agencies and ministries in the course of his or her work.

Uplift professional practice in social work
  • Respond to crisis situations based on organisation's standard operating procedures
  • Identify areas of improvement in models of care and service delivery based on evidence-based practices
  • Support the implementation of standards of care, organisational health policies, procedures and programmes
  • Guide volunteers and placement students in their work
  • Support professional development of staff within the organisation
  • Participate in research to uplift the professional practice in social work
Conduct casework
  • Conduct needs and intake assessment
  • Document and maintain case files and prepare reports according to the required documentation standard
  • Review casework interventions for improvements
  • Conduct closure of cases upon achieving sustainable service outcomes for clients, families and communities
  • Implement case and care plans and interventions
  • Conduct care/ case planning and goal setting based on standard guidelines and professional assessments
Conduct group work
  • Facilitate group work interventions under supervision
  • Plan group work interventions
  • Review group work interventions for improvements
Deliver programmes
  • Assist in the development of programmes through data collection and the conduct of basic analysis, and evaluation of programmes
  • Assist in the implementation of programmes
  • Perform development and planning of programmes to address service gaps
Perform community development work
  • Review community development initiatives for improvements
  • Conduct environmental scans and community need assessments
  • Collaborate with other agencies to pool community resources
  • Support development and implementation of community development initiatives
Perform stakeholder engagement and partnerships
  • Build rapport and professional relationships with clients and significant others
  • Share and apply social work practice across disciplines
