Job description

THKMC was incorporated on 13 October 2011 as a charity to provide multiple social and welfare services to the community at large. It achieved an Institute of Public Character (IPC) status on 18 November 2011. THKMC is the charitable arm of Thye Hua Kwan Moral Society, which lives by the mission To Serve Mankind to help anyone who needs help regardless of their race, religion, colour, language, creed, and culture. THKMC serves more than 70,000 beneficiaries through over 70 programmes across five Services, which include Disability, Early Intervention for Children, Family, Seniors' Community, and Therapy.

Responsibilities

Key Responsibilities: To conduct case management and psychoeducation, and to plan and facilitate groupwork for residents at THK Homes for Disabled.

Clinical aspects

  • Manage enquiries and referrals
  • Conduct assessment and home/site visits, and provide counselling, behavioural interventions, groupwork and support for residents and their families
  • Conduct comprehensive assessments of residents' care and financial needs, and support in the submission of HOMES means testing and transport subsidy applications as required
  • Look after clients' physical and emotional welfare, identify anomalies and provide the appropriate intervention or service linkage
  • Manage emergency response and crisis management with regards to residents' behaviour management with the multi-disciplinary team
  • Develop, implement and review Individual Training Plans for residents with the multi-disciplinary team
  • Provide information, referral and psychoeducation services to caregivers
  • Network and collaborate with relevant stakeholders and community partners
  • Participate in multidisciplinary team meetings and case conferences as required and actively share information in a respectful, non-judgemental manner

Process and Resources Aspects

  • Maintain accurate and timely updates of case recordings, client profiles and other related case work management processes
  • Maintain accurate records, update records and prepare reports for reporting to governing agencies and government means-testing applications
  • Adhere to the standard operating procedures and social work practices
  • Plan and implement projects/programmes assigned
  • Attend and participate in individual clinical and group supervision sessions
  • Attend relevant professional development seminars and training programmes for self-development

Qualifications

  • Minimum Degree or Post Graduate Qualifications in Social Work from a recognised institution

  • Proficient in Microsoft Office and administrative skills
  • Passionate and committed to working with people with disabilities and their families
  • Good team player with a positive and resilient attitude
  • Prior experience with persons with disability will be appreciated
  • Able to work on weekends or weekday evenings, when necessary, e.g., events, roadshows that support the Disability Culture
