About The Role

• Direct work with patients and families, including intake interviews, conducting financial assessments, providing necessary assistance, and assisting with referrals to facilitate discharge planning in specific areas of care.
• Support Medical Social Workers in case management through translation assistance, patient escort, and home visit support.
• Participate in work process improvements and quality improvement projects.
• Collaborate with other hospital departments and external organizations to facilitate patient care and enhance experience.
• Assist in the administrative and operational functions of the department, including managing the counter and related duties.
• Support departmental programs and projects.

What We Are Looking For

• The role reports to a Medical Social Worker and works with the Team Leader for Social Work Coordinators.
• A minimum qualification of a Diploma is required; a Diploma in Social Work or related fields is preferred.
• Good interpersonal and problem-solving skills are essential.
• IT knowledge and skills are necessary for the role.
• Bilingual proficiency is crucial.
