The Salvation Army, Children and Youth Group - Gracehaven is a residential home for children and youths who require care, protection and rehabilitation due to risk behaviours and a lack of parental supervision.
We aim to help children and youths reintegrate back into their family and society through an array of holistic programmes and activities specially designed to cater to the wellbeing of each individual person. At Gracehaven, a team of professionally trained social workers work towards building holistic and individualized care plans that are tailored to meet each person's needs.
As a Social Work Associate, you will be next-in-line to coordinate and support in general administration for the daily business of the Residential Services. You shall work hand-in-hand with the Residential, Programme and Social Work teams to care for children and youth at risk. You will be supporting Assistant Director, Residential in various collation and report submissions for various stakeholders / agencies.
1) ADMINISTRATION:
1.1 Coordinate and support in Quarterly Audit checks by MSF, Annual Muster Audit inspection and Licensing Inspections.
1.2 Assist in the preparation of all statistics, information and reports as required by The Salvation Army and government and ensure that they are collated accurately and submitted timely to designated authorities.
1.3 Provide support in general administration, e.g: creating/updating/filing of resident's documents in main files and by adhering to Personal Data Protection Act policies, etc.
1.4 To maintain the archives of resident's files and data.
1.5 To attend to any enquiries with appropriate customer service etiquettes.
1.6 Participate in meetings, record minutes and minor admin support to management team.
1.7 Preparation of meeting room(s) for meetings / case conferences as and when required in support for management team.
1.8 To support Social Workers that all clients' outstanding medical bills are settled before SW does case closure for respective discharge cases.
2) SOCIAL WORK: (when required support by Social Work Manager, Residential)
2.1 Support Social Workers in house visits, supervise assess either phone or physical visits between residents and parents/caregivers.
2.2 Contact clients and follow up on aftercare light touch.
2.3 Advise clients and their families on general resources available.
2.4 Support to arrange appointments with clients.
2.5 Ensure Social Workers recordkeeping and residents' data is up-to-date in a timely manner.
3) OTHER DUTIES:
3.1 To participate in trainings for self-development and professional growth and to constantly work towards servicing the interest and the needs of our residents better.
3.2 Stand in for Care Staffs to take care of residents in case of emergency or staff meetings/ training.
3.3 Stand in for Care Staffs / Social Workers to bring residents for any resident-related appointment (Eg: sending/fetching residents to/from school, medical appointments etc).
3.4 Stand in for Programmes Dept when extra support might be needed.
3.5 Undertake any other duties as and when required and in consistent with the position.
Diploma from a recognised institution, Diploma in Social Services preferred.
44 hours per week.
Under normal circumstances the hours of duty will be as follows:
10.00am to 7.00pm - Monday to Friday (1 hour lunch break).
The program is a 24/7 community care service, hence flexibility in work arrangement is expected.
