Job summary

SAMH SAY-IT! (Sports, Arts, Youth Integrated Team) seeks a Social Work Associate to support youth mental health services and outreach. You will conduct screening, assessments and case management while coordinating with families, schools and hospitals.

You will work as part of a multidisciplinary team, providing counselling and follow-up support, and may participate in community outreach and programme activities.

Qualifications

  • Minimum Diploma in Psychology or related discipline.
  • 1–2 years of relevant work experience preferred.
  • Experience working in Social Service Agencies especially youth work is an asset.
  • Experience with facilitation, case management and counselling.
  • Proficient in MS Word, Excel and PowerPoint.

Job description

SAMH SAY-IT! (Sports, Arts, Youth Integrated Team) provides assessment and intervention for youths and engages youths for ongoing monitoring and support. YIT also works actively with youth agencies, schools and community partners to provide holistic case management for youths and their families.

Job Purpose

The Social Work Associate supports the YIT team to ensure service delivery is in accordance to the intended programme objectives and care model for YIT programme as outlined under the terms & conditions of the Letter of Award/Addendum. This role requires the identified personnel to have the relevant competencies to support clients with mental health issues.

He/She provides individual support to clients through screening, assessment, intervention and case management. He/She will work closely with the YIT team to achieve the YIT goals of outreach to increase mental health awareness, promote early identification and link those at risk and with needs to the appropriate support and resources and provide follow up support.

In addition, the Social Work Associate will also work closely with YIT team in networking with other community partners or agencies, for purpose of outreach to the youth population.

Job Responsibilities
A). Client Management
  • Screen referred young persons for mental health distress.
  • Provide individualised, comprehensive, community-based services and support to youth and their families.
  • Implement case management service model in working with youth and their family.
  • Conduct intake assessment for youth referred to the centre.
  • Assist family in crisis preparation and safety planning.
  • Formulate individualised and family care plans.
  • Work closely with families, schools and hospitals and job places to support reintegration of client into community setting.
  • Work with families to improve knowledge of mental illness and ability to handle crisis or relapse.
  • Provide counselling and support to youth with emotional, or psychological difficulties.
  • Ensure that all clinical forms and case recordings are up to date.
  • Manage, supervise and motivate the participants of the programme.
  • Conduct support groups and experiential group workshops for youths and their families.
  • Participate in internal, as well as external case conference with community agencies, i.e. hospitals, schools and social service agencies as needed.
  • Work as part of a multi-disciplinary team within organisation and also with external partners and community agencies as needed.
  • Provide after office hours support to youth and caregivers when necessary.
B). Outreach
  • Outreach work in collaboration with centre team (include giving psychoeducation talks, presentations, workshops, information booth, networking with community resources and schools etc).
  • Involve in outreach initiatives to raise mental health literacy in young people and the general public.
  • Network with key community partner sand hospitals and discuss with YIT team to ensure smooth working relationships and referrals with partners.
C). Administration and Evaluation
  • Administer all assessment and evaluation to youths and caregivers and ensure proper documentation.
  • Assist in revising guidelines, standards and procedures, systems and databases.
  • Assist Head and Admin Officer in Annual Report or statistics collation when necessary.
D). Operations
  • Manage day-to-day operations of YIT centre (e.g. opening and closing of YIT centre, stock-taking, maintaining walk-in area and displays as well as overall crowd control).
  • Report and follow up on any faulty equipment or furniture promptly to ensure smooth operation of the centre.
  • Oversee the upkeep and maintenance of YIT office premises and facilities to ensure a safe and conducive working environment.
E). Other Duties
  • To work on ad-hoc SAMH projects, community outreach programmes and events such as World Mental Health Day, YOUthAlive! etc.
  • To carry out any other duties assigned by the Head and Chief Executive Officer.
Qualifications & Experience
  • Minimum Diploma in Psychology or related discipline.
  • Minimum 1 to 2 years of relevant work experience is preferred.
  • Experience working in Social Service Agencies especially youth work, will be an added advantage.
  • Experience with facilitation, case management and counselling.
  • Must be proficient in MS Word, Excel and Powerpoint.
  • Experience with and having skills in the arts (visual arts, music, drama, dance etc) and/or sports will be an added advantage.
