Institution: National University Polyclinics

You will be the key contact officer for the Medical Social Service to attend to all telephone and email enquiries. You will provide administrative support to the Medical Social Worker on referrals, accompany social worker on field visits and assist in interpretation if required. You will also gather information and conduct preliminary investigation on patients' needs to facilitate intervention by Medical Social Worker. You will also process application and ensure submission are in compliance.

You are responsible for maintaining the work statistics and compile the data for submission to Ministry of Health. You will also handle external correspondences such as MP letters and CDC letters. You are also expected to participate in projects that improve the work processes in the department and perform any other duties as assigned by your supervisor.

Job Requirements:

  • Diploma / Higher Nitec / GCE 'A' or 'O' level.
  • Certificate in community and social services or equivalent will be preferred.
  • At least 1 year of working experience in community health.
  • Possess good interpersonal and communication skills.
  • Fluent in local dialects will be an advantage
  • Proficient in Microsoft Office applications such as Word and Excel.
  • Demonstrate ability to be empathetic, non-judgmental, flexible, matured, having a desire to help, and be independent
  • Willingness to travel between polyclinics and may be required to do home visits to ascertain client's home environment.
