Job summary

Changi General Hospital Pte Ltd. is seeking a dedicated nurse to coordinate patient care and discharge planning across the multidisciplinary team. You will educate patients and caregivers, ensure continuity of care, and document care plans accurately.

Job Requirement:

• Possess a Advanced/Higher/Graduate Diploma, Bachelor's Degree, Post Graduate Diploma, Professional Degree, Nursing or equivalent

• Minimum 5 years’ experience in acute care

• Demonstrated leadership and care coordination capabilities.

• Strong stakeholder management and interdisciplinary collaboration skills.

• Experience in quality improvement projects and mentoring junior staff will be an advantage.
