Job summary

Qualifications

• Possess Degree in Social Work or Degree with a Graduate Diploma in Social Work from an accredited institution
• At least 4-5 years of relevant experience
• Accredited as Registered Social Worker with Singapore Association of Social Workers would be an advantage

Responsibilities

• Prepare case notes and documents for assigned cases
• Provide guidance on needs and intake assessments for complex cases
• Formulate casework intervention plans
• Implement case interventions and care plans / programmes
• Prepare case reports (e.g., social reports, Multidisciplinary Team Meeting Reports, Internal Case Review Reports)
• Review reports, escalation reports and close cases when sustainable outcomes are reached
• Manage crisis cases with partner organisations and agencies
• Review models of care and service delivery aligned to evidence-based practice
• Review casework intervention plans and supervise junior colleagues
• Evaluate progress and intervention effectiveness
• Provide individual and group staff supervision
• Conduct environmental scans and community need assessments
• Formulate and facilitate group work interventions and/or community development
• Collaborate with community partners to execute programmes
• Assist staff with implementing self-care plans
