Job summary
An established organization in dementia care seeks a dedicated professional to assess and support individuals with dementia and their caregivers. This role involves conducting assessments, providing interventions like psychoeducation, and facilitating support group sessions. The ideal candidate should hold a relevant degree and possess strong interpersonal and communication skills, with a passion for dementia care.
Qualifications
- Preferably with a degree in an allied health profession, social work, or psychology.
- Ability to facilitate group sessions will be an added advantage.
- Prior working experience in dementia care is a plus.
Responsibilities
- Conduct case assessment and formulate Individualized Care Plans for Persons with Dementia.
- Provide psychoeducation, supportive counselling, and service referral.
- Plan and facilitate Caregiver Support Group sessions.

Responsibilities
- Conduct case assessment and formulate Individualized Care Plan for Persons with Dementia and their carers.
- Provide intervention such as psychoeducation, supportive counselling and service referral. Able to administer Advance Care Planning will be a bonus.
- Liaise and collaborate with multidisciplinary professionals and external service providers for holistic care management.
- Provide case recordings and relevant documentations.
- Participate in team meetings and case conferences as required.
- Assist in the preparation of data and statistics for programme evaluation and reporting purposes.
- Plan and facilitate Caregiver Support Group sessions.
- Support in outreach events and talks.
- Any ad-hoc tasks as and when required.
Requirements
- Preferably with degree in an allied health profession/social work/ psychology etc. Senior position may be considered for this role.
- Good oral, written and interpersonal communications skills.
- Ability to facilitate group sessions will be an added advantage.
- Able to work on occasional Saturdays and weekday evenings.
- Passionate about dementia care and working with people.
- Prior working experience in dementia care will be an added advantage.
