Job summary

Wecruit Pte Ltd is seeking a committed caregiver to assist in developing individualised care and support plans for residents according to assessed needs and home procedures.

The role supports implementation of care plans, monitors residents' progress and reports changes to the appropriate personnel. You will organise activities to promote well-being and quality of life, and conduct monthly resident interviews with records for about 50 residents.

Job description

• Assist in developing individualised care and support plans for residents according to their assessed needs and the Home’s established procedures.

• Support the implementation of care plans, monitor residents’ progress and report relevant changes or concerns to the appropriate personnel.

• Organise and implement suitable programmes and activities to promote residents’ well-being, engagement and quality of life.

• Conduct monthly resident interviews and maintain accurate records, including six-monthly review reports, for approximately 50 residents.
