Job summary

CHIEF RECRUIT PTE LTD in Singapore seeks a qualified physiotherapist to assess clients at admission, during reviews, and at discharge. You will set SMART goals, plan evidence-based, person-centered treatments, and document assessments, plans, interventions, goals, and outcomes clearly.

Collaborate with clients and the team to advance care, supervise Therapy Assistants, train on assistive devices where needed, and contribute to quality improvements and program development.

Qualifications

• Diploma or Degree in Physiotherapy.
• Dynamic and proactive personality with good public relation and communications skills.
• Possess good management, excellent analytical and problem-solving skills.

Responsibilities

• Conduct comprehensive physiotherapy assessments during client admission, reviews, and discharge.
• Plan and deliver person-centered, evidence-based physiotherapy treatments.
• Maintain clear and thorough documentation of assessments, treatment plans, interventions, goals, and outcomes.
• Share assessment findings, treatment strategies, goals, progress, and discharge plans with clients and stakeholders.
• Oversee the safe and effective use of rehabilitation tools and provide training on assistive devices.
• Assess and recommend suitable assistive devices and equipment based on client needs.
• Develop discharge plans and continuity of care strategies in collaboration with the team.
• Promote community integration by coordinating with external partners.
• Equip clients and caregivers with self-management techniques for long-term health and function.
• Participate in multidisciplinary case discussions for coordinated care planning.
• Ensure timely documentation and reporting of client care and outcomes.
• Contribute to clinical program development and service improvements.
• Support training, guidance, and supervision of Therapy Assistants.
• Engage in continuous learning and professional development.
• Participate in quality improvement and research initiatives where applicable.
• Adhere to audit standards and regulatory requirements.
• Perform additional responsibilities as assigned by the Reporting Officer.
