Job summary

RECRUIT FAST PTE. LTD. in Singapore seeks a qualified physiotherapist to perform comprehensive assessments during admission, reviews and discharge, and to plan care.

You will set SMART, person-centered goals, deliver evidence-based interventions, document outcomes, and communicate plans to clients and stakeholders. AHPC registration and a physiotherapy degree or diploma are required, with at least two years in neuro-rehabilitation.

Qualifications

• Diploma or degree in Physiotherapy with AHPC registration.
• Minimum 2 years of neuro-rehabilitation experience.
• Good team player and able to work independently.

Responsibilities

• Conduct accurate physiotherapy assessments during admission, care plan reviews and discharge.
• Select outcome measures to evaluate progress and guide clinical decisions.
• Formulate SMART, person-centered goals with clients and stakeholders.
• Design and conduct evidence-based physiotherapy interventions.
• Document assessment results, therapy plans, interventions, goals and outcomes.
• Communicate assessment results, intervention plans, goals, outcomes and discharge plans to clients and stakeholders.
• Ensure proper use of rehabilitation equipment including technology-assisted devices.
• Formulate discharge and continuity of care plans with stakeholders.
• Facilitate community integration through collaboration with community partners as required.
• Empower clients and caregivers with self-management strategies.
