Job summary

Qualifications

  • Strong clinical reasoning skills for mobility, pain, respiratory symptoms and functional decline.
  • Ability to provide patient- and family-centred care and to lead goal setting with patients and caregivers.
  • Experience supervising junior staff or therapy assistants and contributing to training.

Role Description

Provide physiotherapy care to patients with life-limiting illnesses across hospice and community care settings.

Responsibilities

Responsibilities include:

  • Assess functional needs and recommend appropriate equipment and assistive devices.
  • Conduct goal-oriented and collaborative physiotherapy encounters, working with patients and, where appropriate, their families or caregivers to establish meaningful and achievable goals that reflect the patient's priorities, values, and stage of illness.
  • Develop and implement individualised physiotherapy care plans based on collaboratively identified goals and the patient's functional, psychosocial, and palliative care needs.
  • Provide interventions to optimise comfort, mobility, function, participation, and quality of life.
  • Conduct home- and centre-based visits.Maintain accurate and timely clinical documentation.
  • Provide patient and caregiver education, including training in safe transfers, exercise programmes, energy conservation, and the use of assistive devices.
  • Mentor junior therapists and/or therapy assistants.
  • Contribute to clinical governance, service improvement, staff training, quality initiatives, and outreach activities.
  • Work closely with the multidisciplinary team to deliver holistic, patient- and family-centred palliative care.
Qualifications
  • Recognised degree in Physiotherapy and full registration with the Allied Health Professions Council (AHPC), Singapore.
  • Clinical experience in physiotherapy, preferably in acute care, rehabilitation,oncology, geriatrics, palliative care, or community/home care settings.
  • Strong assessment and clinical reasoning skills in the management of mobility limitations, pain, respiratory symptoms, functional decline, and other complex presentations.
  • Ability to provide patient- and family-centred care and to facilitate collaborative goal setting with patients and caregivers.
  • Demonstrated ability to supervise, mentor, and support junior staff, and contribute to clinical education and training.
  • Excellent communication and interpersonal skills for working with diverse patients, families, and multidisciplinary teams.
  • Good documentation, organisational, and time-management skills, with attention to quality and safety standards.
  • Comfortable working in home and community environments, including making independent clinical judgements and adapting interventions to varied settings.
  • Commitment to compassionate care, ethical practice, and respect for cultural and religious diversity.
  • Willingness to participate in continuing professional development and service improvement initiatives.
  • Prior involvement in research, quality improvement, clinical audit, or programme development initiatives would be an advantage.
