Job summary

Worq AI in Singapore is seeking a qualified Pharmacist to join our hospital inpatient team. You will perform medication reconciliation, dispensing, and provide specialised counselling to patients while reviewing orders and discharge prescriptions.

You will also act as a resource for drug information, rotate on-call duties, supervise technicians, and participate in hospital projects. The role requires a Bachelor of Science in Pharmacy, valid practising licence, registration with the Singapore

Qualifications

• Bachelor of Science in Pharmacy required.
• Valid practising pharmacist license required.
• Registered with the Singapore Pharmacy Council.
• Minimum 1 year of inpatient hospital experience.
• On-call duties required, no overnight shifts.

Responsibilities

• Perform medication reconciliation, dispensing and specialised counselling to patients and review of patient’s medication.
• Provide drug information to other healthcare professionals and provide on call services on a rotational basis.
• Ensure performance indicators are met and pharmaceutical care is provided according to hospital SOPs and accreditation/legal standards.
• Review and ensure appropriateness of medication orders and discharge prescriptions.

• Assist in any hospital projects as assigned by Principal Pharmacist and/or Senior Manager.
• Maintain the distribution of drugs and to ensure an accurate inventory control system, including controlled drugs.
