Pharmacist (Outpatient Pharmacy)

About the Role

You will be part of the Outpatient Pharmacy department toby working collaboratively with a multidisciplinary team to promote wellness and ensure quality pharmaceutical care to cancer patients.

Responsibilities

  • Prepare and dispense medications using appropriate technique in accordance with department policies and guidelines.
  • Assure completeness and appropriateness of medication order before preparing and permitting the distribution or dispensing.
  • Provide medication-use education to patient and caregivers.
  • Check accuracy of the work of pharmacy technical and clerical personnel or others in accordance with institutional/departmental policies and procedures.
  • Assist pharmacist in-charge in supervising and scheduling work of subordinate staff in the pharmacy.
  • Ensure the continuity of pharmaceutical care to and from the acute and ambulatory patient care settings.
  • Provide concise, applicable, and timely responses to requests for drug information from health care providers and patients.
  • Ensure accurate charging and collection of fees at the pharmacies.
  • Perform drug ordering and maintains sufficient stock for patients' needs.
  • Participate in process for assessing, managing and reporting medication errors.
  • Participate in departmental performance improvement program.
  • Collect and prepare workload statistics and other reports as assigned by Senior Pharmacists/Pharmacy Manager.
  • Perform any duties as assigned by Pharmacy Manager

Job Requirements

  • Bachelor of Science (Pharmacy) or equivalent.
  • Registered with Singapore Pharmacy Pharmacy Council.
  • Strong oral and writing skills
  • Good interpersonal and communication skills, team player
  • Computer literate with knowledge of Microsoft Word, Excel and Powerpoint
