Job Responsibilities

• Provide and facilitate socio-economic evaluations to help patients and their families address practical concerns, such as financial matters, which are often an issue at the end of life.

• On-going individual and family counseling through face-to-face sessions.

• Work with the broader community which includes educational, religious, and social service organizations so that patients and their families can achieve the optimal care plan.

Requirements

• Degree or Diploma in Social Work.

• Registered with the Singapore Association of Social Workers.

• Passion to serve and provide good patient care.

• Good interpersonal skills and able to work independently.

• Flexible, adaptable, and able to work effectively in varied settings.
