Job summary

ALL SAINTS HOME in Singapore is seeking a Senior Medical Social Worker to provide psycho-social assessment and interventions for residents and their families as part of person-centric care. The role collaborates with nursing staff and multi-disciplinary teams in a residential care setting, based in Yishun.

You will design psycho-social programs, liaise with external organizations, and assist with discharge planning and funding schemes.

Qualifications

• Degree in Social Work. Preferably registered as Registered Social Worker.
• Minimum 5 years of related working experience. Relevant experiences in Nursing Home/ILTC sector is an advantage.
• Passionate to work with seniors and their families.
• Excellent communication skills and ability to speak local languages.
• Ability to work well in a team.
• Good interpersonal and problem-solving skills.
• A fast and organized worker while able to multi-task.
• Proficient in MS Office.
• This role is based at Yishun.

Responsibilities

• Conduct residents' pre-admission assessment with nursing staff.
• Attend multi-disciplinary meetings with nurses and other healthcare professionals to discuss, propose and coordinate care plans for residents.
• Design and develop relevant programs on psycho-socio care that promote quality care that look at well-being of residents.
• Liaise with organizations that provide services to meet residents’ and their families’ needs and refer them if necessary.
• Develop discharge care plan for clients if their health condition is fit for discharge.
• Identify gaps and streamline work processes within organization and service deliveries related to residents’ care.
• Advocate for changes with relevant stakeholders to enhance the care and well-being of residents.
• Administer renewals of residents' funding schemes when needed.
• Apply for Financial Assistance (e.g. Eldershield IDAPE, MFEC).
• Manage Medifund administration and applications in collaborations with Finance Department.
• Any other duties as assigned by the Reporting Supervisor.
