Job description

Leading, planning and development of social work practice standards, quality outcomes, in support of the organisation's overall mission to empower patients, clients and caregivers in the community through all-round support.

Responsible for overseeing the planning, development, and delivery of social work and mental health services, in line with the organisation's overall mission and care model.

Ensure quality and cost-effective delivery of social work and mental health services in home care, caregiver support and community outreach.

Identify needs and develop appropriate social work services in support of the organisation's patients, caregivers and community.

Review, develop and enhance work processes/guidelines of social work department and CREST team to ensure efficiency and compliance with government guidelines and regulations, such as Means Testing, MediFund and ILTC subsidy scheme

Guide, manage and develop Social Work Department's staff to meet the department's objectives, including managing workload distribution, developing training plans and providing supervision and mentorship to staff in the department.

Collaborate with internal clinical and non-clinical colleagues in clinical care and in support of the organisation's goals.

Identify and develop appropriate networking and collaborative partnerships with external organizations to advance the organisation's mission.

Any other duties assigned by the said reporting officer.

Qualifications

Bachelor of Arts degree in Social Work or its equivalent. A Certificate in Counselling would be an advantage

Registered Social Worker

At least 8 to 10 years of social work experience, with at least 3 years of supervisory experience, and 1 year in healthcare

Able to apply knowledge and experience to address practical problems of staff and clients
