Job description
What The Role Is

You will be part of a dynamic team that drives and coordinates the service and capacity planning, and national implementation of home-based care services.

You will be working in a fast-paced and dynamic environment that would require the ability to manage multiple priorities and stakeholders at the same time.

What You Will Be Working On

In this role, you will be involved in the policymaking, planning and implementation of home-based care services.

This Will Involve
  1. Reviewing, developing and implementing policies and services to transform home-based care, so as to allow our growing population of seniors to receive seamless holistic care and age-in-place in the community, and avoid institutionalisation. This includes projecting of future demand and capacity required, developing new care models for greater quality of care and efficiency of delivery, and putting in place enablers (e.g. IT, data-sharing, appropriate financing approach).
  2. Ensuring programmes are run according to the policy objectives and guidelines, e.g. by reviewing operators’ performance and facilitating follow-up action where necessary.
  3. Monitoring and analysing population trends and citizen feedback, and conceptualising, developing and evaluating pilots to ensure services continue to meet evolving needs.

  8. Relevant working experience in healthcare, social services and/or policy formulation is an advantage.
  9. Excellent interpersonal skills and able to work with and bring on board multiple stakeholders with diverse interests.
  10. Good written and verbal communication skills.
