RESPONSIBILITIES

Roles & Responsibilities:

Secretariat and Coordination Function

  • Support care integration work in Toa Payoh subregion with partner organisations, including SCC, AAC, HPC+, and HT providers, and to achieve the deliverables specified for the Integrated Community Care Provider.
  • Work with ground teams to implement the sub-regional workplan, and monitor and report on the progress of the workplan.
  • Coordinate and facilitate regular meetings and check-ins for partner organisations to build understanding and work through implementation and other challenges.
  • Engage and build networks and partnerships with other community care providers, regional health systems, grassroot organisations and other stakeholders.
  • Assess resources and identify opportunities for care integration and coordination collaborations across ICCP partners and with external partners
  • Support in the development and trial of joint referral workflow with ICCP partners to support care coordination for seniors.

Administrative Function

  • Work with the AIC's sub-regional Relationship Manager and partners on the reporting and monitoring of indicators.
  • Collect and consolidate the information required for the quarterly progress report
  • Ensure that the endorsed report and claim form are submitted to AIC in a timely manner
  • Support in meetings convened by AIC as required, and report back to the leadership team of the partner organisations

Additional Responsibilities

  • Take on additional tasks and responsibilities as required by the management to ensure the continued success of the ICCP function in the sub-region.
QUALIFICATIONS

Possess a Bachelor's degree from an accredited University, preferably in Social Work, Community Work, Gerontology, or related field.

Requirements:

  • At least 3 years of experience in eldercare coordination, community care, or a related field
  • Experience working with seniors or in senior care services
  • Good understanding of community care delivery, integrated service models, and inter-agency collaboration
  • Good understanding of the senior care landscape and relevant industry regulations
  • Good communicator and strong interpersonal skills to build partnerships and work with cross-functional teams
  • Strong organization skills and structured
  • A proactive, solution-oriented mindset focused on delivering results.
