Job Description:

• Responsible for day-to-day operation of Health Programme for cardiology patients

• Provide clinical and administrative support, including screen and recruit suitable cardiology patients into the programme, coordinate care with clinicians and primary care partners

• Arrange and follow up on appointments, and provide patient support and health counselling

• Track and document each recruited patient’s progress and ensure compliance of the clinical protocol

• Prepare reports and timely submission to the Head of Department

• Any ad-hoc duties as per assigned

Requirement:

• Bachelor’s degree in Allied Health, Life Sciences, Nursing, or a related field

• Minimum 3- 5 years working experience in healthcare settings
