Job description

Overview

The Position Holder will be responsible for engagement of partners and internal stakeholders to augment NUHS’ role as the population health manager for the western region. This includes ideation and project management of collaborations with partners and within NUHS to support the roll out of new policies, advancing social-health integration and enhance the delivery of care in the community.

Job Responsibilities

You will be responsible for the following:

  • Identify opportunities for strategic collaborations with key partners to improve care integration and augment community care
  • Supporting implementation of NUHS Population Health strategies and regional health plan
  • Regular engagement and project management with key account agency/community/ internal partners on collaborations
  • Keeping abreast of developments within social and health landscape
  • Support team in development, implementation, data collation, evaluation and reporting of initiatives and collaborations with agency partners, social care players and cross-teams within NUHS
  • Meeting and event organisation
  • Taking minutes for meetings and events and following up on issues raised

Job Requirements

  • Good Degree
  • 1 - 2 years of working experience, prior experience in community care or strategic planning will be an advantage
  • Experienced in project management and working with multiple stakeholders
  • Good communication, interpersonal and organisational skills
  • Ability to multi-task and work independently
  • Ability to work in a fluid environment amidst multiple demands
