Job summary

CELECTI PTE. LTD. is seeking an experienced Senior Dietitian to join a leading public healthcare organization in Singapore. You will deliver person-centred nutrition care, conduct home visits, and work across community posts including active ageing centres.

Key responsibilities include nutrition assessment and care planning, medical nutrition therapy, telehealth delivery, and collaboration with multidisciplinary teams to improve community health outcomes.

Qualifications

• Bachelor/Master of Science in Nutrition and Dietetics required.
• Experience in community health nutrition, clinical dietetics, and program development preferred.
• Proficiency in client assessment, care planning, and telehealth service delivery.

Responsibilities

• Responsible for all phases of nutrition care including nutrition assessment, diagnosis of dietary/nutritional problems, prescription of appropriate dietary treatments and nutrition care plans
• Provide medical nutrition therapy to individuals, families, and at-risk populations via home visits, telehealth, and outreach
• Collaborate with multidisciplinary teams to develop, implement, and evaluate nutrition programs to improve community health outcomes
• Monitoring and counselling of patients in a variety of settings
• Provide advice/ coaching to caregivers in terms of nutrition care for the patients

• Working hours: 8am - 5:30pm (Mon-Thurs); 8am – 5:00pm (Fri)

You will provide person-centred care to patients in the areas of nutrition care, working across various community posts such as active ageing centres (located in central/ central North) and conduct home visits.
