You will manage patients with complex care needs, maintaining comprehensive records of demographics, medical histories, and treatment plans whilst ensuring seamless care coordination and continuity. You will collaborate with multidisciplinary teams including physicians and allied health professionals to coordinate care plans, facilitate timely follow-ups, and manage referrals to community resources. You will also collect treatment data and facilitate meetings with various stakeholders. Additionally, you will organise population health initiatives for the community and ensure all programmes comply with relevant regulations and standards.
