You will coordinate care and case management for manage patients with complex care needs, maintaining comprehensive records of demographics, medical histories, and treatment plans while ensuring seamless care coordination and continuity. You will collaborate with multidisciplinary teams, including physicians and allied health professionals, to coordinate care plans, facilitate timely follow-ups, and manage referrals to community resources. You will also support population health research through data collection, collect treatment data and facilitate stakeholder meetings, and with various stakeholders. Additionally, you will organise population community health initiatives for the community and ensure all programmes comply with relevant regulations and standards , ensuring all works meet regulatory compliance.

Requirements
