Senior/Care Coordinator (COMIT)

Responsibilities and Duties:

·The Care Coordinator is to conduct home visits to provide psycho education, support and linkages cum care-coordination of services to clients and caregivers.

·Conduct assessment of patients for medical, psychological, social, environmental and vocational needs.

·He/She has to be a team player to collaborate with a group of Allied Health Professionals, doctor and nurses.

·Provide psychological input and expertise during multi-disciplinary meetings.

·Provide trainings to team / caregivers as necessary.

·To conduct Mental Health support group for patients and /or caregivers.

· To carry out projects and duties assigned by supervisor or Head of Department.

Job Requirements:

·Registered Nurse with Singapore Nursing Board.

· Possess a RMN/Adv Dip in Mental Health or other relevant qualifications (e.g. social work, counseling, psychology).

·Experienced in psychiatric setting for at least 3 to 5 years (Community psychiatric clinic / IMH) will be advantage.

·Has experience using psychotherapy with mental health patients.

·Passion to work in the community.

·Possess strong organizational, interpersonal, communication and writing skills.

· Good computer literacy skills – MS Office etc.

·Familiar with healthcare environment.

·Ability to multi-task, work independently and collaborate as a team.

·Will be advantage to be able to converse with English, Chinese, Malay & Dialect-specking clients.

·Passion in managing people with mental health disorders.
