The incumbent will be part of the Transitional Care and Community Partnership team at TGCH and play an important role in shaping and delivering population health strategies that centre around building community capability and supporting individuals in navigating care transitions across settings.The incumbent will work with people and communities to uncover local strengths, resources and networks that support health and wellbeing. She/he will work closely with clinician leads, community partners, social service agencies and residents to create pathways that promote continuity of care, strengthen informal and community-based support system.
