Job summary

Cadmus Resources is seeking an experienced physician to lead care planning for residents in a geriatrics setting. You will collaborate with a multi-disciplinary team to develop and review care plans, and respond quickly to changes in residents' health status.

The role requires a basic medical degree and full registration with the Singapore Medical Council, plus experience with elderly patients. You will educate residents and support advance directives while guiding nursing staff on care

Qualifications

• Basic medical degree recognized by the Singapore Medical Council.
• Full registration with Singapore Medical Council.
• Experience with elderly patients or graduates in geriatric or family medicine is advantageous.
• Excellent communication and interpersonal skills.

Responsibilities

• Develop care plans with a multi-disciplinary team and review goals for residents.
• Respond promptly to changes in residents' function or condition.
• Inform residents of health status and support self-determination including advance directives.
• Conduct rounds with the staff nurse in charge to ensure plan implementation.
• Conduct regular resident check-ups, diagnose ailments, and refer for specialist care when needed.
• Formulate enhanced care philosophy for nursing staff.

Responsibilities:

• Work together with a multi-disciplinary team to develop care plans for residents, periodically review them and assure that the goals for each care plan are rational and relevant.
• Respond in a timely fashion to resident's change in function or condition.
• Inform residents of their health status and enable residents to exercise self-determination including advance directives.
• Conduct rounds with the staff nurse in charge to ensure care plans are being carried out.
• Conduct regular resident check-ups and diagnosis of ailments, prescribe medication and refer resident(s) for further specialist check-ups where required.
• Formulate enhanced care philosophy for nursing staff.

Requirements:

• Basic medical degree recognized by Singapore Medical Council.
• Registered with Singapore Medical Council with full registration.
• Relevant experience working with elderly patients, or those with Graduate Diploma in Geriatric Medicine / Family Medicine or relevant disciplines will be advantageous
• Possess good communication and interpersonal skills.
