Job description

Responsibilities:

  • Attend to general telephone enquiries and provide information to the public with regard to the proper procedure for application into Lions Home for the Elders (LHE).
  • Regularly update and declare the number of occupancy beds in the Bright system for our 2 Homes (Bishan/Bedok).
  • After assessment of referrals, inform the Medical Social Worker (MSW) and AIC Team on the outcome of the assessment. Schedule the date of admission into LHE (Bishan/Bedok).
  • To prepare the necessary documents for the CEO to sign after a new admission is completed.
  • To Coordinate with the Medical Social Worker (MSW) of referring hospitals for all relevant documents and make arrangements for the transfer of residents from hospital/TCF/home, and schedule the date of admission into into LHE (Bishan/Bedok).
  • Prepare and fill up the admission form. Inform the respective ward level, Director of Nursing, and staff concerned before the date of admission of residents.
  • Meet with the Next-Of-Kin (NOK)/Guarantor to explain the details of the written agreement with LHE.
  • Provide emotional support to residents and their families, and manage residents’ transitions to LHE.
  • Address complaints or concerns from residents, families, or the community in a timely and professional manner.
  • Mediate onflicts between residents, families, or staff to ensure a harmonious environment within the nursing home.
  • Contact NOKs with financial hardship and work out plans for instalment payments.
  • Assist residents’ family members or NOKs to apply for Long-Term Care Schemes if required. E.g.: IDAPE, PGD, Eldershield, CareShield Life, Medisave Care.
  • Assist in applying for the Medifund scheme for residents if NOKs need financial assistance.
  • Schedule the Medifund/FAS quarterly meeting with Medifund Committee Members.
  • Oversee the Medifund audit with the help of department staff.
  • Check the accuracy of the submission of half-yearly and yearly Medifund/FAS statistical reports to MOH by the Executive, Public/Social Welfare.
  • Oversee the annual ILTC audit.
  • Attend monthly MCM meetings.
  • Cover colleagues’ duties when colleague(s) is on leave.
