Responsibilities and Duties:

• The Care Coordinator is to conduct home visits to provide psychoeducation, support, and linkages for care coordination of services to clients and caregivers.
• Conduct assessments of patients for medical, psychological, social, environmental, and vocational needs.
• Collaborate with a team of Allied Health Professionals, doctors, and nurses.
• Provide psychological input and expertise during multi-disciplinary meetings.
• Provide training to team members and caregivers as necessary.
• Conduct mental health support groups for patients and/or caregivers.
• Carry out projects and duties assigned by the supervisor or Head of Department.

Job Requirements:

• Registered Nurse with the Singapore Nursing Board.
• Possess an RMN/Advanced Diploma in Mental Health or other relevant qualifications (e.g., social work, counseling, psychology).
• Experience in a psychiatric setting for at least 3 to 5 years is advantageous.
• Experience using psychotherapy with mental health patients.
• Passion for community work.
• Strong organizational, interpersonal, communication, and writing skills.
• Good computer literacy skills – MS Office, etc.
• Familiarity with healthcare environment.
• Ability to multi-task, work independently, and collaborate as a team.
• Passion for managing people with mental health disorders.
