As a Programme Executive for our Fun with Friends Programme, you are a dedicated care team that delivers a person-centred care model for our seniors, including persons with dementia. We aim to promote seniors' independence and maintain their cognitive and physical functions through multi-modality group programmes, ensuring they lead fulfilling lives.

You will need to:

  • Plan and facilitate various multi-modality group activities for persons with dementia; including exercise sessions, meal preparation groups, cognitive games, music and art-related activities
  • Manage clients with different mental, and physical conditions and behaviour of concerns relating to dementia and assist in the monitoring of client's health status
  • Assist in client's care including providing necessary assistance for transfers, toileting and functional mobility
  • Administer assessment to clients and caregivers, perform data collection and entry
  • Provide timely updates to family/NOK of client's engagement and progress in the programme
  • Provide customer support to customers, partners and other external stakeholders, which includes but not limited to enquiries, enrolment, service coordination and feedback
  • Support and conduct training for community care associates and volunteers and assist in supervising community care associates in ensuring the smooth running of the programme
  • Administrative tasks such as managing client's database, assisting with procurement-related matters, transport claims, monitoring and tracking budgeting and expense records, and planning staff coverage and roster
  • Provide basic housekeeping of the programme premise
  • Ensure timely and effective communication between internal and external stakeholders (e.g. therapist, exercise trainers and activities facilitators, programme team, volunteers and vendors) in designing and running collaborative programmes
  • Prepare timely reports to the management and funders of the programme and assist with internal and external audits to ensure the programme meets the service model requirement
  • Assist in outreach and networking events to promote the program, generate new interest and recruit new client

Qualifications

  • Candidates who possess a Diploma in social service or a related discipline
  • Candidates who possess working experience in eldercare, health care, and therapy-related areas possess an added advantage

You should:

  • Be skilled in facilitating group sessions and programme
  • Be empathetic and form positive relationships with persons with dementia and caregivers
  • Thrive in a collaborative and dynamic environment
  • Demonstrate strong interpersonal and problem-solving skills Be proficient in English and Mandarin.The ability to converse in dialects will be an added advantage.
  • Be proficient in Google Suite Applications/MS Office

Working hours:

  • Five-day work week 8-5.30 pm, Timing may vary according to programme needs
  • Region-wide- West, North and East region, but might be required to move around for role
