Job description

Job Purpose:

To design, coordinate, and implement meaningful day care and rehabilitation programmes for seniors in the Senior Care Centre. The role aims to promote physical, cognitive, and social well-being while ensuring a safe and person-centred environment. The Programme Executive also supports clients' functional needs in collaboration with the care team.

Key Responsibilities:
1. Programme Planning & Execution
  • Plan and implement daily programmes and activities tailored to seniors' needs (e.g., exercise, art & craft, reminiscence, games).

  • Facilitate group activities and provide individualised engagement where necessary.

  • Collaborate with therapists to deliver rehabilitation activities or cognitive stimulation exercises.

2. Client Care & Supervision
  • Supervise seniors during activities, meals, and rest time to ensure safety and comfort.

  • Provide basic assistance with mobility, toileting, feeding, and transfers if needed (especially for frail or dementia seniors).

  • Observe and report any changes in seniors' conditions to the reporting officer/ centre manager.

3. Documentation & Reporting
  • Maintain accurate and timely documentation of programme attendance, senior progress, and activity outcomes.

  • Assist in collecting data for reports, centre reviews, and outcome measurements.

  • Participate in care reviews and communicate with family members as directed by the reporting officer/ centre manager.

4. Environment Management
  • Ensure activity areas are safe, clean, and well-prepared before and after programmes.

  • Manage and maintain inventory of programme materials and supplies.

  • Support centre-wide events and festive celebrations.

5. Interdisciplinary Collaboration
  • Work closely with the centre manager, nurses, care executives, therapist, and volunteers to deliver holistic care.

  • Assist in the onboarding and supervision of care executives and volunteers participating in centre activities.

6. Family & Community Engagement (Optional)
  • Engage with caregivers and family members to provide updates and promote involvement in care.

  • Support community outreach efforts, including weekend events and activities, health screenings, and intergenerational activities.

7. Any Other Tasks

  • Perform any other administrative or operational tasks as assigned by the Reporting Officer, Centre Manager, or Management.

Qualifications & Requirements:
  • Diploma in Community Care, Social Work, Psychology, Gerontology, or related discipline.

  • At least 1–2 years of experience in eldercare, social services, or programme coordination (preferred but not mandatory).

  • Passionate about working with seniors, including those with dementia or physical limitations.

  • Creative, energetic, and resourceful in activity facilitation.

  • Strong interpersonal and communication skills.

  • Able to work both independently and as part of a multidisciplinary team.

  • First aid certification or eldercare-related training (e.g., Eldercare WSQ) is an advantage.
