Are you passionate about mental health advocacy and making a real impact in the community? Join CREST (Community Outreach Team), a vital safety network supporting individuals at risk of or living with mental health conditions, including dementia, and their caregivers.
As a Program Executive, you'll play a key role in outreach, care coordination, and community engagement to promote mental wellness and support those in need. If you're a proactive team player with a heart for mental health, this could be the perfect opportunity for you!

• Plan, implement, and oversee outreach initiatives, care processes, referrals, and follow-up workflows to ensure individuals receive the right support.
• Document and evaluate programme progress to assess effectiveness and identify areas for improvement.
• Develop service planning documents and operational manuals, including setup requirements and process guidelines for the CREST programme.

Community Engagement & Outreach
• Conduct needs assessments to identify service gaps and enhance mental health support within the community.
• Build partnerships and collaborate with agencies, organizations, and community groups to co-develop and implement mental wellness programmes.
• Stay updated on mental health trends and community needs to inform service improvements.
• Conduct outreach to corporates and community organizations to promote mental wellness, raise awareness, and share relevant resources.
• Organize and facilitate education and training programmes to enhance knowledge of mental health and coping strategies.

Client Support & Care Coordination
• Conduct mental health screenings using validated assessment tools to support early intervention.
• Provide care coordination and service linkages for identified clients, ensuring they receive the necessary support from referral to follow-up.
• Offer mental health information and emotional support to at-risk individuals and their caregivers.
• Monitor client progress through regular follow-ups and re-evaluations to ensure continued well-being.
• Provide clinical supervision to team members.

Data Management & Reporting
• Compile, analyze, and maintain data to track programme impact and prepare reports as required.
• Assist in identifying trends and making data-driven recommendations to enhance service delivery.

Qualifications
• A Bachelor's degree in Social Work, Counselling, or a Master's degree in Psychology from a recognised university.
• At least 3 - 5 years of relevant experience in the mental health, social services, or community outreach sector.
• Experience in supervising a small team and providing guidance or clinical supervision to team members.
• A strong interest in mental health advocacy and supporting individuals with mental health conditions.
• Good communication skills to engage with different stakeholders and build meaningful connections.
