Principal Social Worker

Main Responsibilities

• Leading social workers and/or associates and oversees their case management. Assign cases/referrals based on case intensity (risk & needs matrix), case loading capacity and appropriateness in terms of individual skillsets and competencies.
• Leading social workers in driving the standard of ethical and clinical practice, as well as social work competencies through clinical supervision, training, therapy groups and case consultations.
• Leading social workers in fronting our clinical practice and practice standards to other organisations, both social work and non-social work related.
• Plan, develop and implement clinical programmes with evidence-based methodologies, and validate processes and outcomes through rigorous evaluation.
• Leading social workers in social work events, both internally and externally, that enhances organisation sectoral influence among social workers and healthcare professionals.
• Leading social workers in conducting research that have significant impact in terms of renal social work sector. Research outcomes to be presented through sharing platforms such as journals and seminars.
• Lead and collaborate with healthcare professionals internally and externally in holistic patient management.

Main Responsibilities

• Provide case management support to social workers in areas of client engagement, conducting interviews, data collection, case assessment (BPSS, risk & needs assessment and other relevant assessment tools), case conceptualisation, case planning and intervention plans for best patient outcomes.
• Structure and maintain standards in processes within clinical supervision, training, therapy groups, and case consultation. To monitor via timely evaluations and updates to the management.
• To profile and demonstrate the efficacy and outcome of Community Care efforts through advocacy platforms within healthcare and the social work sector.
• Develop several clinical programmes alongside social workers that will provide impactful outcomes to patients and stakeholders. Ensure successful and timely delivery of programmes with validated evaluative reports.
• To be involved in the provision of influence, via internal and external webinars, with direct impact on the standing of social workers. Involve and guide social workers in the provision of abovementioned activities.
• Guide research work within the social work team, and work alongside other stakeholders in areas of collaborative research, be it internal or external partners, as directed by Function Lead and/or Director.
• Integrate social work assessment and interventions into a holistic care model, with enhanced collaboration with other disciplines.
