Qualifications

  • Minimum 1 year of experience in physiotherapy or occupational therapy.
  • Registered with the Allied Health Professions Council in Singapore.

Responsibilities

Job description

Job Responsibilities
  1. Conduct initial assessment / follow-up reviews / other specific assessments of designated patients in day rehab and day care.
  2. Develop individual care plans based on patients' needs, adopting a multi-disciplinary team-based approach and implement appropriate interventions / activities.
  3. Review and re-assess individual care plans regularly and/or when there is a change in patient's condition for both day rehab and day care clients.
  4. Attend multi-disciplinary round and present cases for discussion / sharing when necessary.
  5. Make referrals to other service providers, when required and as agreed by the patient or next-of-kin.
  6. Provide certification on patients' fitness for rehab for rehab services for self-referral and walk-in clients.
  7. Support HNF's initiatives/projects related to Therapy services.
  8. Provide physiotherapy/occupational therapy treatment for clients at Senior Care Centre.
  9. Document all inputs in a timely manner.
  10. Plan and conduct purposeful activities for clients at the senior care centre.
  11. Develop Care staff and Therapy associates abilities in conducting suitable exercises and engaging activities.
  12. Any other related duties as directed by the said reporting officer.
Qualifications
  1. Minimum Bachelor's degree in Physiotherapy/Occupational Therapy with at least 1 year of experience.
  2. To be registered with the Allied Health Professions Council in Singapore as Full Registrant.
  3. To have a valid BCLS certification.
