Job summary

Worq AI is seeking a licensed pharmacist to review transfer medications, counsel patients, and verify new orders. The role involves discharge prescription review, medication supply duties, and follow-up on preparation status, with both in-person and virtual counselling options.

Requires a B.S. in Pharmacy, Singapore registration, and excellent communication and teamwork skills. Weekday and weekend shifts may be available.

Qualifications

• Bachelor of Science in Pharmacy from an accredited college by Singapore Pharmacy Council.
• At least 2 years of relevant experience.
• Fully registered with the Singapore Pharmacy Council.
• Possess excellent communication, interpersonal and organisation skills.
• Able to work independently and in a team.
• Weekdays and/or weekends working hours are available.

• Review transfer medications appropriately, contact patients pre-transfer on medication supply needed, and answer medicine related questions
• Medication counselling on transfer medications by a variety of means (in-person and virtual)
