Job Description:

  • Dispensing and educating patients on their medications, medication review and reconciliation, therapeutic interventions and recommendations, multidisciplinary rounds and collaborative prescribing for qualified persons.

  • Recommend and monitor drug therapy to achieve desired outcomes.

  • Provide drug information to other healthcare professional and patients.

  • Identify, resolve and prevent drug-related problems.

  • Provide pharmaceutical care, working collaboratively in a multidisciplinary team in various clinical settings, including outpatient, inpatient and community settings.

  • Ensure pharmacy policies and procedures are implemented consistently.


Requirement:

  • Degree in Pharmacy or equivalent / SPC registered

  • Good interpersonal and communication skills.

  • With good knowledge of Microsoft Offices
