Job summary

Ren Ci Community Hospital in Singapore is seeking a Pharmacist to provide safe, accurate dispensing and collaborate with a multidisciplinary team to ensure patient safety and regulatory compliance.

Qualifications

  • Bachelor of Pharmacy from a recognized institution.
  • Registered with the Singapore Pharmacy Council with a valid Practising Certificate.
  • At least 2 years of relevant hospital/clinical/retail experience.

Responsibilities

  • Review, validate, and process medication orders to ensure safety and accuracy.
  • Manage medication-related issues in collaboration with prescribers and teams.
  • Conduct medication reconciliation during transitions of care to prevent errors.
  • Perform medication reviews and optimize outcomes through pharmacotherapy.
  • Counsel patients and caregivers on safe and effective medication use.
  • Support chronic disease management with monitoring plans.

Responsibilities
  • Provide safe, accurate, and timely medication dispensing services in compliance with regulatory, legal, and professional standards.
  • Review, validate, and process medication orders and prescriptions to ensure appropriateness, completeness, accuracy, and patient safety.
  • Manage and resolve medication-related issues in collaboration with prescribers and multidisciplinary healthcare teams.
  • Conduct medication reconciliation during transitions of care to ensure accurate medication records and minimize medication errors.
  • Perform medication reviews and identify, prevent, and resolve medication-related problems to optimize patient outcomes.
  • Develop and implement pharmaceutical care plans based on patients’ clinical conditions and therapeutic needs.
  • Provide medication counselling, education, and support to patients, caregivers, and healthcare professionals to promote safe and effective medication use.
  • Support chronic disease management by providing evidence-based pharmacotherapy recommendations and monitoring plans.
  • Conduct discharge medication counselling, facilitate self-medication programmes, and ensure smooth transition of care for patients returning to the community.
  • Proactively identify and resolve post-discharge medication-related issues and address patient feedback and concerns effectively.
  • Support inventory management activities, including medication receipt, storage, stock monitoring, replenishment, expiry management, and disposal in accordance with regulatory requirements.
  • Participate in procurement activities, vendor management, formulary evaluations, medication utilization reviews, and cost-containment initiatives.
  • Generate and analyze reports using pharmacy and clinical information systems to support operational efficiency and service improvement.
  • Serve as a pharmaceutical resource for healthcare teams by providing drug information, clinical advice, and medication‑related support.
  • Train, mentor, supervise, and support the development of pharmacy staff, students, volunteers, and other healthcare professionals.
  • Participate in quality assurance, patient safety, quality improvement, and research initiatives to enhance pharmacy services and patient care outcomes.
  • Assist in the investigation of medication incidents, inventory discrepancies, product recalls, and implementation of corrective actions.
  • Identify, assess, and report risks related to pharmacy operations and support Ren Ci Hospital’s risk management framework through proactive risk mitigation and adherence to organizational policies and procedures.
Requirements
  • Degree in Pharmacy from a recognized institution.
  • Registered with the Singapore Pharmacy Council with a valid Practising Certificate.
  • At least 2 years of relevant experience; prior hospital, clinical, or retail pharmacy experience will be preferred.

Workdays
  • Monday: 8.30am to 5.15pm (includes one hour lunch break)
  • Tuesday to Friday: 8.30am to 5.30pm (includes one hour lunch break)
  • Saturday: Once every 4 weeks, 8.30am to 12.30pm
  • Total 40.75 hours per week
